New Patient Registration
You will need to fill in a questionnaire and a registration form and ideally bring with you your NHS number, a valid photo ID such as a passport or driving licence photocard and a proof of address.
Please bring your documentation to the Practice for staff to process your registration. You will also have the option to book for a new patient check when you register.
From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a named accountable GP.
This is purely an administrative exercise in order that patients can have a named responsible GP should they require them.
Should you wish to discuss this procedure, please speak to the receptionists or Clinician when you are next in the surgery.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
Please note: there is no need to telephone the practice for this information.
The Practice has facilities for disabled patients.
Changing Your Personal Details
If you change your name, address, post code or telephone number, please give full details to reception.
Remember to tell us if details for other members of your family have also changed.
